How to make a claim?

Corporate Business Travel Accident

Corporate Business Travel Accident has similar claims procedure with our Travel Guard’s line of products. However, if your employees have to make a claim, they have to talk to their HR personnel or to your insurance broker so that these authorized personnel can assist you in submitting the necessary documents.

1.    Fill out the Travel claim form from the AIG Philippines website.

2.    If the person Insured gets into a personal accident or wants to reimburse a medical expense, submit the following documents:

  •  Medical report from physician or hospital complete with admitting medical history, diagnosis, course in ward
  •  Original copy of the prescriptions, official receipts and bills of medical expenses incurred
  •  Police report (in case of accident)

       A.  In the event of death or repatriation, please report to Travel Guard Asia Pacific and bring the following:

  •   Death certificate
  •   Port mortem Report
  •   Birth certificate
  •   Marriage contract

       B.   During a cancellation trip or curtailment, please bring these documents:

  •  Tour operator’s cancellation notice
  •  Certification stating the reason for the trip cancellation/curtailment
  •  Documents to support the trip cancellation/curtailment
  •  Official receipts of expenses incurred due to trip curtailment
  •  Official receipts of payments made in advance for trip cancellation

       C.  During a flight delay, submit these papers:

  •  Certification from the airline/carrier stating scheduled departure time, actual departure time and the reason for delay of the flight
  •  Official receipts of expenses incurred due to flight delay

       D. For baggage delay, please submit these:

  •  Property irregularity report
  •  Proof of acknowledgement of baggage receipt
  •  Official receipts of expenses incurred due to baggage delay

       E.  For baggage and personal effects, bring the following:

  •  Receipts of lost/damaged items being claimed
  •  Photos of damaged luggage/items
  •  Property irregularity report from airline/hotel

Group Personal Accident

Group Personal Accident has similar claims procedure with our Individual Personal Accident. However, if your employees have to make a claim, they have to talk to their HR personnel or to your insurance broker so that these authorized personnel can assist you in submitting the necessary documents.

1. Fill out the Accident/Sickness Insurance claim form from AIG Philippines website.

2. Submit the following documents:

  • Accident/Sickness Insurance Claim Form 
  • Attending physician’s report form
  • Police investigation report or statement of witnesses
  • Copy of the certificate of insurance
  • Proof of premium payment

       A. For accidental death, submit the following:

  • Birth certificate of the Insured and Beneficiary(ies) –  authenticated copy
  • Death certificate – authenticated copy
  • Marriage contract – authenticated copy
  • Autopsy report
  • Other documents that our claims department may require

       B. For dismemberment/disablement, submit:

  • EENT specialist’s certification in case of loss of sight
  • Medical specialist’s certification on the medical/physical condition of the Insured        
  • Photos showing the latest condition of the Insured             

       C. For medical reimbursement, please submit:

  • Medical certificate
  • Medicines doctor’s prescription
  • Official receipts for medicine, laboratories, professional fee
  • Hospital statement of account
